The concept of Community Health Workers (CHW) is an emerging profession. A CHW works within diverse communities who are at great risk of developing health complications, chronic health illnesses or early death. They provide a wide range of services such as outreach, home visits, problem solving, connections with social service and other activities. More importantly, a CHW advocates for their client’s success to obtain quality health in a timely manner and help eliminate health inequities. The mission of Community Partners is to build a pathway that will provide evidence based mechanisms for other community providers and service organizations who would like to join our endeavors in creating a healthier community utilizing the Health HUB.

Available at the Community Partners web site is a Request for Proposal for organizations to complete, if they would be interested in contracting a CHW with Buffalo County Community Partner’s Health HUB program.
